Why this structure
Good meeting minutes are a decision record, not a transcript. Every field above exists to answer one question a reader will actually ask later: what did we decide, who owns what, and what's still open. Skip anything that doesn't serve that — a play-by-play of the conversation is what a transcript is for, not the minutes.
